Running-in of M/E main, crankpin, and crosshead bearings

What are the most common defects observed in M/E main, crankpin, and crosshead bearings after shop test, and sea trials?

Scratches caused by dirt / foreign particles in oil would get my vote for the most common defect after sea trials.
Usually shipyards rush at the LO flushing and commissioning stage of M/E.
